Appearance

E-221 is a smiling face, with 2 large eyes that connect into the mouth. In game, it is tinted blue.

Mechanic

once E-221 Spawns, it will very quickly rush to the current room, where it will stop for ~4 seconds before shooting back to it's spawn location. It will repeat this a few more times, before rushing back to the current room to despawn.
